When a business receives a Chamber Cash Certificate, the business shall fill in the bottom portion on the back of the certificate and mail the certificate to New Lisbon Chamber, 119 E. Bridge Street, New Lisbon, WI 53950. The Chamber will issue a check for the full face value of the certificate.

You may not adjust the amount that is printed on the certificate

There is no cash value for this certificate for the customer
Each business will decide how it is rung into their register

Certificates are valid for one year from the date they are issued. The date is hand written on the front of the certificate

The "Presented to" portion of the certificate does not need to be filled in
The bottom portion on the back of the certificate must be filled in by the business before it is sent to the Chamber office
